UnitedHealthcare Reform News: United eServices SBC Request Form Available on Sept. 23


The Summary of Benefits and Coverage (SBC) is intended to provide consumers with a concise document explaining, in plain language, simple and consistent information about health plan benefits and coverage. It will summarize the key features of the plan, such as the covered benefits, cost-sharing provisions, and coverage limitations and exceptions.

UnitedHealthcare will distribute the SBC to members according to the SBC rules. Note: In the pre-enrollment situation, UnitedHealthcare will not know the identities of eligible but unenrolled members so we will rely upon our employer customers, or broker acting on their behalf, to deliver the SBCs to new hires mid-year, and at enrollment.

 

Effective September 23, 2012 you can request a Summary of Benefits and Coverage document for your client or broker. Simply log in to www.unitedeservices.com and select "Summary of Benefits and Coverages Request Form” under the "Quick Links" section on the United eServices ® Home page. Complete and submit the form and your Summary of Benefits Coverage document(s) will be emailed to you within seven business days.
